How affirmative consent works at our counter.

Most customers enroll online, but an active Maui Pack and Ship mailbox holder can also enroll in Package Alerts in person, using a paper consent form. This page documents that process and shows the exact form we use to record it.

Package Alerts are optional. Enrollment is never required to obtain, rent, or maintain a Maui Pack and Ship mailbox, or to purchase any goods or services. Customers who don't enroll continue to receive our standard notification: a blue package notice card placed inside their mailbox when a package is ready for pickup.

The Process

In-Person Opt-In Workflow

This is the exact process our retail staff follow when a customer asks to enroll in Package Alerts in person, at 415 Dairy Road, Suite E, Kahului.

    Customer requests enrollment at the counter

    An active mailbox holder tells staff they'd like to receive Package Alerts text notifications instead of, or alongside, our standard physical notice card.

    Staff provide the paper consent form

    The "MPS Package Alerts™ — Optional In-Person SMS Consent Form" shown below. It states the program is optional before any information is collected.

    Customer completes their information

    Full name, mailbox number, and mobile number — plus business name and email if they choose — filled in by hand, at the counter.

    Customer reviews and affirmatively checks the SMS consent box

    The checkbox is never pre-checked. The customer reads the consent language — message purpose, frequency, data rates, STOP/HELP — and must actively check it themselves.

    Customer initials and signs

    A separate acknowledgment specific to SMS consent, confirming enrollment is voluntary and not required for mailbox service.

    Maui Pack and Ship retains the record and activates separately

    Staff record the signed consent. Package Alerts membership ($8.99/month) and account activation are completed separately, in house — signing the form does not itself authorize payment.
